Who is eligible?
Children between the ages of 6 and 18 who have experienced the loss of a parent, or close supporting family member.

Who can refer a child?
Referrals can come from anyone. In many cases referrals come from relatives, teachers, school counselors, social workers, a parent or legal guardian.

How do I refer a child?
You may use the electronic referral form, or download and print a form. A downloaded form can be mailed or faxed.

What support does Parent Plus provide?
Parent Plus strives to provide children opportunities for personal growth and development by funding and facilitating participation in established artistic, athletic, or academic programs.

How does the process work?
A completed referral form is submitted to Parent Plus.
The referral person is contacted by Parent Plus.
The parent or legal guardian (if different than the referral person) is contacted, and further information regarding the family situation is requested.
A program or activity for the child is identified, and a goal is established.
The parent or legal guardian enrolls the child in the program or activity and provides Parent Plus with a copy of the application.
Parent Plus contacts the program or activity provider and delivers funding.
A follow up plan is established.

How long does it take?
Application to enrollment generally takes 2-6 weeks.

How long can a child be involved with Parent Plus?
Funding can be one-time, or on a continuous basis. Length and depth of participation is at the discretion of the Board of Directors.
